Individual Health Care Plans

     If a student has a medical condition that will require monitoring or interventions during the school day, an individual school health care plan will be needed. The school health plan will be developed by the school nurse, the student’s physician and family.

     The parent or guardian of a student with a chronic medical condition is required to provide the school with a Health Care Plan as soon as possible and have it updated yearly.

     Examples of medical conditions include but are not limited to:

          Asthma

          Diabetes

          Allergic reactions
