beginning in 2016 many Americans will be receiving a new IRS form that contains new information needed when you file your taxes this form pull up form 1095 contains information on your health insurance so what is this form the quick answer is the affordable Care Act which has two main requirements it requires large employers like your school system to offer a certain level of minimum affordable health in nice to their employees the good news is that the coverage offered by State Health Benefit Plan those requirements the second requirement is that most of men's must attain health insurance either through an employer again like your school system or from some other approved Source like a state or federal Marketplace Exchange employers and insurance companies are required to verify to you and the federal government that employees have been offered this required coverage and as employees have to verify that we have taken insurance so form 1095 is the new Federal Form that gives you the information you need when you fired tax return beginning with your 2015 tax returns this form contains information on the employer coverage offered in the employees coverage information including your spouse in any cupboard that you have on them as with other tax forms penalties will be assessed for those employers are insurance companies I do not supply this information if you change the employer's here in the year or had insurance coverage from different employers you may receive multiple form 1095-c the information received on form 1095 will simply be transferred to your tax returns in the same way you do your W2 information if you have already filed your taxes in are receiving this form you do not have to file an amended return for 2015 simply keep this information with your tax records please talk to your tax professional for more detailed information on these forms
